Service Description
Medical & Preventive Services (By Appointment Only) Preventive Health Consultation: $200 -Comprehensive health review -Preventive screening recommendations -Review of available laboratory results This service focuses on preventive guidance and health planning and does not replace a traditional annual physical exam. Ongoing Health Support: $150 / month Personalized medical support for common health concerns, including: -Blood pressure monitoring -Metabolic health -Thyroid support -Cholesterol management -Weight-related health concerns -Medical Consultations In-person consultation (limited availability): $125 Telehealth consultation: $100 Visit type is determined based on clinical appropriateness. Prescriptions: Weight-management medication refills. Included in active programs Prescription refill requests (outside active programs): $50 Laboratory Services: -Laboratory testing is not included in visit fees. -Tests are billed directly to the patient by Quest Diagnostics or Ulta Lab Tests, according to their pricing.
Cancellation Policy
📅 HOURS OF OPERATION Monday – Sunday 10:00 AM – 6:00 PM By appointment only. Same-day appointments may be available depending on staff availability. ________________________________________ 📘 1. BOOKING POLICY Appointments Required All IV therapy services require a confirmed appointment. Walk-ins are not guaranteed. Arrival Please arrive 10 minutes early to complete any required forms and assessment. Late Arrivals Clients arriving 10 minutes or more late may experience a shortened session or may need to be rescheduled. Medical Screening All clients must complete a health screening and consent form before receiving IV therapy. ImmunoCare IV Studio reserves the right to decline treatment for safety or contraindications. ________________________________________ 📕 2. CANCELLATION POLICY Cancellation Window We require at least 24 hours’ notice for cancellations or rescheduling.
